This form is opened when you select the Create option from the Customer Satisfaction Index survey administration form. The survey creation wizard guides you through the process of creating questions for and naming the survey. There are five main steps to the creation process, however three os the steps are repeated for each question which is added to the survey. The stage group seen on the left hand side of the form will indicate the stages which have been completed and the stage you are on.
The wizard steps are:-
The Welcome form simply welcomes you to the wizard and advises that each of the steps are should be completed carefully. There are no fields on the form which require completion it is purely for information
This stage of the wizard enables you to chose the type of question you want to create. There are up to two fields on this form which require completion.
- Please select question type: enables selection of the type of question from the drop down. the question types are:-
- True/false: this type of question is worded in such away that the customer can only answer yes or no. An example of this could be Were you greet by a member of staff as soon as you entered the dealership?
- Lickert: this type of question can be used where there are several questions about the same subject which need to be asked. The customer is asked to grade their answer.
- Multiple choice: multiple choice questions enable you to ask a question which could have more than one response, the customer who is being surveyed will be asked to select the one which most closely matches their experience.
- Free text: free text questions enable you to get general feed back from the customer these are very often the "have you any other comments?" type questions there are no structured or guided responses to this type of question.
- Please enter the number of allowed options: this field is only enabled on multiple choice and Lickert questions it requires you to enter the number of possible responses.
The question details enable you to enter the question to be asked the question weighting and any additional required information. The fields requiring completion will vary according to the question type selected. Details about the information required for each question type can be viewed below.
Fields for this question type are:-
- Question: the question text
- Weight: The importance of the question for this CSI.
- True text: the text which will be used against the true option, for example Yes
- False text: the text which will be used against the false option, for example No
Fields for this question type are:-
- Question: the text for the question to be asked. Allows up to 99 characters
- Weight: the importance of the question for this CSI.
- Statements: the options you require rating by the customer
- Scale headings: the options for the ratings for example excellent, very good etc.
Multiple choice questions allow the customer to select from more than one answer.
- Question: the text for the question to be asked.
- Weight: the importance of the question for this CSI.
- Choices: the statements available for the customer to choose
The required information for this question type are:-
- Question: the text for the question to be asked
- Weight: the importance of the question for this CSI.
The review question form enables you to check the question set up before confirming it.
As with the question details stage the information displayed on this form will vary according to the type of question being created you will be able to see all of the information entered against the question regardless of the question type.
NOTE: The question type, question details and the review question forms will be repeated until the finish option is selected.The save survey form enables you to specify the ID and description for the survey. The two fields on this form are:-
Push buttons
- Survey ID: the three character identifier for the survey
- Description: the description for the survey
The button labels my change according to the stage you are at in the survey/question set up. Possible push button options are:-
- Exit: closes the Survey wizard and returns you to the CSI survey administrator form
- Help: opens the linked html help page.
- Previous: moves you one stage back in the process
- Next: moves you forward in the wizard to the next stage
- Next question: takes you back to the question type stage to begin the question creation steps for a new question.
- Finish: opens the saving survey stage of the wizard
- Save: saves the survey and returns you to the CSI survey administrator form.
