This form is displayed on selecting the Damage code field's Ellipsis button on the Ford Warranty Claim Information form's Repair details tab.

Location:

Broadly identifies where on the vehicle the damage occurred:

• Left side

• Right side

• Interior

• Front end

• Rear end

• Undercarriage / Roof / Misc.

Note: Location constitutes the first two digits of the resulting composite Damage code on the Ford Warranty Claim Information form's Repair details tab.

Tip: Selecting a Location radio button displays a default Location code.

Location code:

Identifies where on the vehicle, within the area defined by the selected Location, the damage occurred.

Note: The Location code displayed depends on the selected Location radio button:

Location	Location code default value
Left side	10 Door/shuts. Left front
Right side	09 Door - Cargo light
Interior	26 Headliner
Front end	01 Aerial
Rear end	01 Bumper/cover//extension - rear
Undercarriage / Roof / Misc.	02 Battery

Tip: You can refine Location code further by using the down-arrow button .

Damage code:

Damage code: 00 to 38

Note: This form's Damage code constitutes the second two digits of the composite Damage code on the Ford Warranty Claim Information form's Repair details tab.

Severity:

Severity: 1 to 6

Note: Severity constitutes the last digit of the composite Damage code on the Ford Warranty Claim Information form's Repair details tab.
