This form is opened when you select the Customer Satisfaction Index (CSI) survey setup option from the CRM miscellaneous option menu. It enables access to the a suite of functions which guide you through the process of maintaining CSI surveys. As this forms purpose is to enable access to the survey maintenance functionality the tool and menu bars are limited.

The available surveys grid displays the surveys that the customer has completed. It is split into four columns:
- ID: Three character identifier for the survey.
- Description: Full identity of the survey
- Survey status: Current status of the survey this could be
- Saved: Survey for which the questions have been created and that has been given an ID and description.
- Complete: Survey for which the available department(s), locations(s), and contact code has been defined.
- Published : Survey which has been finalised and release for use in CRM.
